When do immunizations need to be turned in to NYOS Nurses?
  • Within 30 days of admission to NYOS Charter School.
  • When you get a new immunization at a yearly check-up.
 
When do affidavit's need to be turned in to NYOS Nurses?
  • Within 30 days of admission to NYOS Charter School.
  • Every 2 years if it is an affidavit of conscience.
  • If it is a lifetime waiver then only once on admission.
Note: When turning in an affidavit, NYOS must receive the original copy, not a copy of the original copy. To learn more about requirements for obtaining an Immunization Exemption/Affidavit, please go here.
