Next, the Student Foreign Exchange organization should provide NYOS a School Placement Packet via email which will include the following type of information:

School Acceptance form - for Principal signature and date agreeing to enroll the student

Student name, nationality, academic year, host family contact information, school contact Information

Student bio - timeframe, sex, country of birth, home address, email, dob, photo, hobbies/interests

Parents/Legal guardian info/relationship - first/last name, phone, email, occupation

Guardian release form (signed and dated by student and parents)

General consent form (signed and dated by student and parents)

Academic agreement (signed and dated by student and parents)

Program rules (signed and dated by student and parents)

Grade report card with translation

Teacher recommendation

Medical history
-
Physical examination

English language test for international students

Immunization record
