Completing Forms
Every year NYOS will notify you when new forms have been added or if you are required to complete more information. Forms appear at the end of the icon list on the dashboard.
Reviewing Form History
To get a history of completed forms.

Click the "hamburger menu" icon

Select "Online Form"

In the FEATURES pop out click "My Online Forms History"
