1. Click the down arrow to the right of the "Home" icon.
2. Select "New Student Enrollment"
   

Every year NYOS will notify you when new forms have been added or if you are required to complete more information. Forms appear at the end of the icon list on the dashboard.

Form History

To get a history of completed forms.

1. Click the "hamburger menu" icon
2. Select "Online Form"
3. In the FEATURES pop out click "My Online Forms History"

It is important we know the most up-to-date information regarding your student. Make sure to review your family information regularly and make changes when necessary.

1. Click the "hamburger menu" icon
2. Select "Student"
3. In the FEATURES pop out click "Student Profile"
   
4. Click on the pencil icon next to the "YOUR FAMILY INFORMATION" header
   
5. After making the necessary changes click the "Save" icon/button at the TOP of the form (weird, right?).
