When do immunizations need to be turned in to NYOS Nurses?

• Within 30 days of admission to NYOS Charter School.
• When you get a new immunization at a yearly check-up.

Affidavits

When do affidavits need to be turned in to NYOS Nurses?

• Within 30 days of admission to NYOS Charter School.
• Every 2 years if it is an affidavit of conscience.
• If it is a lifetime waiver then only once on admission.

Note: When turning in an affidavit, NYOS must receive the original copy, not a copy of the original copy.

CDC Recommended Immunization Schedules

The following schedules are provided by the CDC and are updated annually.

• Birth Through 6 Years Old
• Children 7–18 Years Old
• Adults Aged 19 Years and Older
