1. Complete Registration

Find the correct registration form for your sport below. You'll need your registration fee receipt in order to submit.

2. Turn in your Physical Evaluation

In order to participate in any NYOS sport you must get a physical evaluation by a doctor.

• Print and take Physical Evaluation Form to your appointment to have your doctor complete and sign. 
• Email completed form to [email protected]

3. You Made It! Pay for Your Sport

Now that you've made the team (congrats!) you'll need to pay your sport fee. Click your sport below to make your payment.
